Did you receive written notice with a Notice Number that begins with the letter P?: If you have already received written notice by e-mail which contains a Notice Number that begins with the letter P, then, in order to obtain a payment, you must submit a Short-Form Claim Form.

The e-mail notice you received will have your Notice Number listed (begins with the letter P).
